San Simeon by the Sound is proud to announce that its long awaited Adult Day Health Care/Medical Model program will be opening this May.  We will be accepting applications in March and April for those interested in utilizing our program to enhance the lives of the people they care for and to keep your family together longer.

• Program hours: 9am-3pm, five days a week, Monday through Friday
• Professional Nurse on duty overseeing the care needs of the registrants
• Medical care, as prescribed by your M.D., including medication administration as necessary as well as skin care
• Registered Dietitian overseeing your nutritional needs
• Daily care met by our experienced certified nurse assistants
• Recreation program
• Physical and Occupational Therapy when prescribed by your M.D.
• Case management, by our Social Worker, to help you to access the resources available to you in your community to manage healthcare
• Health Education by professional staff to help you manage your health care needs in a holistic manner
• Beautician
• Continental breakfast; hot lunch, afternoon snack
• Transportation will be available.

The costs for the program are eligible for Medicaid payment as well as Long term Insurance and private pay.
