Is there a waiting list?
The availability of beds is hard to predict. San Simeon has a very dynamic rehabilitation program with people coming, getting better and leaving all the time. So, beds for short-term rehabilitation are frequently available. There may be a wait for a private room or a specific unit. Submitting an application is always recommended. Applications can be reviewed and kept on file until an appropriate bed becomes available. Applicants can then be notified and the admission process completed.
What is required to apply?
Three forms are needed to complete the application package: The application (which provides demographic information, emergency contacts, insurance coverage, and income and asset information), a Patient Review Instrument or PRI (which provides current medical condition information) and a Patient Screening Instrument or Screen (which confirms the need for skilled nursing services). The applicants, or their designated representative, are responsible for submitting the application. PRIs and Screens are submitted by referring hospitals and facilities. Applicants living in the community can obtain PRIs and Screens through a certified home healthcare agency or other qualified evaluators. Click here to download a pdf of the application.
How long does the application process take?
A complete application package can be reviewed very quickly. A determination is usually made the same day a complete package is submitted. Once an application is approved, arrangements for admission are begun immediately, but arrangements care be made to hold a bed.
Are Medicare and Medicaid accepted?
San Simeon by the Sound is a certified Medicare and Medicaid facility. Private funds, private insurance, long-term care insurance and some healthcare maintenance organization policies are also accepted.
