
Adult Medical Day Care Facility
Statistics show that aging adults who require extended healthcare assistance are demanding care outside of the institutional setting. Additionally, shifting demographics and lack of adult day healthcare services within a 50 mile radius of the North Fork area supports alignment with this demand.
San Simeon is proactively addressing this as an extension of our commitment to servicing the community. We will support aging members of our community who desire to maintain a level of independence and want to stay in their homes during times of healthcare crisis or when additional care becomes necessary.
San Simeon is reconfiguring unused space in direct response to this need and will renovate a section of the current facility where long term care rooms will be renovated to fit the needs of 30 people with a wide range of medical conditions associated with aging. Like most other adult medical day care programs, San Simeon’s program will assist individuals with a variety of chronic illnesses, such as arthritis, heart conditions, high blood pressure, diabetes and cognitive deficits, as well as people with Alzheimer’s disease.
The Adult Day Care renovation will also provide a multi-purpose room, dining room, physical therapy room and functional home-like kitchen. A quiet room, exam rooms, conference room, reception area, locker area, as well as showers and bathrooms are additionally included in renovation of this area.
This will result in a program which offers a high quality full-day service under one roof, accommodating the schedules of working families who need assistance in supporting aging family members. As always, our facility and staff will be dedicated to training and supporting community caregivers in addressing problems at home, so that care between home and the center will be consistent. Services include chaperoned ‘door-thru-door’ transportation between home and the center, a staff trained to care for dementia using behavior modification techniques, case management and assistance with toileting and personal hygiene.
Through a single point of entry, clients and their families will gain access to coordinated caregiver support, healthcare, rehabilitative services and respite care.
